As a Care Coordinator, your focus will be on delivering high-quality nursing services, mentoring care staff, and conducting thorough assessments of new residents. You will prioritize individual care needs and develop Person Centre Service Plans, while ensuring adherence to Alberta Health Services regulations.

Key Responsibilities:
• Provide quality nursing care using established processes
• Supervise and educate care staff to improve service
• Perform thorough assessments of incoming residents




• Develop individualized service plans for each resident
• Conduct monthly audits to ensure compliance and quality

Requirements:
• Must hold a current Licensed Practical Nurse certification
• Experience in long-term care settings is advantageous
• Leadership skills for guiding care teams
• Knowledge of Point-Click-Care is valuable
• Certification in First Aid and CPR required
