Be a key player at DischargeHUB as a Patient Transition Liaison, driving patient care excellence through and productive discharge processes. This full-time remote and office hybrid position is based in St. John's.

You will oversee the management of service requests while engaging directly with healthcare stakeholders to ensure that patients transition smoothly from hospitals into their homes. With responsibilities ranging from administrative tasks to communication with diverse healthcare providers, your role will require both attention to detail and exceptional problem-solving abilities. You will also contribute to ongoing improvements within the team.

Key Responsibilities:
• Promptly generate service requests through various channels
• Liaise with stakeholders to address their service request concerns




• Keep meticulous records and documents of service requests
• Collaborate on the development of operational procedures
• Introduce suggestions for enhancing service efficiency

Requirements:
• Diploma or Bachelor's in Healthcare Administration or similar
• Experience in service management or customer service
• Strong interpersonal and communication skills
• Capable of strategic thinking and problem-solving
• Familiarity with healthcare compliance standards is advantageous
