Key Responsibilities:
• Establish rapport with clients facing complex challenges
• Conduct thorough assessments and craft personalized support plans
• Provide behavioral support using a trauma-informed approach
• Assist individuals in navigating community services effectively
• Collaborate with a range of community partners for continuity of care
Requirements:
• Degree or diploma in Social Work, Psychology, or equivalent
• At least three years’ experience in a related field
• Crisis intervention training such as ASIST or Mental Health First Aid
• Familiarity with local mental health and housing support systems
• Proven ability to build trust and engage clients
