Connective is currently hiring Support Workers to enhance the daily lives of individuals in their care. You will work collaboratively in a unionized environment, engaging directly with residents to monitor their progress and advocate for their needs. Your hands-on approach will help create a nurturing atmosphere conducive to personal growth.
Key Responsibilities:
• Conduct interviews to formulate personalized care plans
• Liaise with multidisciplinary teams to adapt treatment plans
• Oversee daily support activities and meal preparation
• Document all case notes and maintain client records
• Provide emotional support and life skills training
Requirements:
• Social work or community social services diploma (an asset)
• At least 1 year of relevant experience
• Robust interpersonal and crisis intervention skills
• BC Class 5 driver’s license required
• Current First Aid Certificate required
