• Accept service requests and conduct client assessments
• Collaborate with community services for appropriate referrals




• Monitor clients’ goals and provide regular updates

Requirements:
• University Degree or College Diploma (Social Work preferred)
• 5+ years experience in developmental services
• Knowledge of community support services
• Robust communication and organizational skills
• Valid Ontario Class G Driver's License and insurance
