In this permanent position, you will serve as a key contact for clients and manage their integration into healthcare services after a vision loss. Your responsibilities will include assessing clients' needs, ensuring timely access to resources, and facilitating communication between service providers. Key Responsibilities:
• Organize and schedule therapy appointments with service providers
• Solicit client feedback to enhance service quality




• Provide proactive follow-ups to maintain client connections
• Explore recent care options tailored to client needs
• Ensure documentation meets VLRC standards

Requirements:
• Proven experience in client service within health sectors
• Relevant diploma in Social Services or similar
• Strong interpersonal and communication skills
• Understanding of client-centered care methodologies
• Experience with vision loss support systems
