This position tasks the Outreach Worker with providing essential support to adults in crisis. You’ll engage with clients to assess their needs, develop action plans, and coordinate access to vital services. Key Responsibilities:
• Provide support and skills training for independent living
• Develop individualized crisis intervention plans
• Assess client needs and create action strategies




• Maintain records and statistical data accurately
• Compliance with health and safety regulations

Requirements:
• Diploma or Certificate; or 2 years relevant experience
• Must have Occupational First Aid Level 1
• Class 5 Driver’s License with transparent abstract
• Strong organization and communication skills
• Criminal Record Clearance for Vulnerable Sector
