We are looking for a compassionate, relationship-focused Care Consultant who loves connecting with people and building community partnerships. You will be the bridge between our clients, families, community partners, and care teams, helping individuals access the support they need while making sure the highest standards of care are maintained. You are great at building trust, solving problems, and finding smart ways to guide people through one of the most important decisions of their lives, across St. John’s Metro and surrounding area.

What You’ll Do





Conduct in-home site visits focused on care monitoring of ongoing home care.

Provide community rectifications and ongoing delegations with Home Support Workers in the community.

Educate clients, families, and community partners about available home care services and supports.

Monitor client satisfaction, identify concerns early, and communicate service or quality issues to the Care Manager for timely follow-up.

Provide coaching, mentorship, and support to Home Support Staff in the community, including assisting with delegated tasks where appropriate.

Maintain accurate documentation and communicate effectively with clients, families, and interdisciplinary team members.

Collaborate with your Care Manager to ensure care is delivered safely, consistently, and in accordance with professional and organizational standards.

What You Bring

Nursing Registration: Registration as an LPN or RN in good standing is considered a strong asset.

Relevant Experience: Experience in home care, community health, case management, or client care coordination is considered an asset but not required

People-First: Excellent interpersonal, communication, and relationship-building skills.

Sharp Judgment: Strong assessment, critical thinking, and problem-solving abilities.

Self-Starter: Ability to work independently while managing multiple priorities.

Good with Tech: Comfortable using technology and electronic documentation systems.

On the Road: Valid driver’s license and reliable vehicle, with the ability to travel throughout the community.
