As we prepare to open the Ng Siu Chan Centre, our newest long-term care home, we are seeking passionate and dedicated Social Worker who want to help shape the culture, resident experience, and care environment from day one. Built on the principles of Relationship-Centred Care and Meaningful Living, the Ng Siu Chan Centre will provide an innovative care environment where residents are supported to live with purpose, dignity, independence, and choice. This is an exciting opportunity to contribute to the opening of a new home and help create an exceptional experience for residents, families, and team members.
Responsibilities:
Using professional case management practice, carries out intake assessment, service coordination, referrals, case conferencing and case monitoring
Provides supportive counselling to clients and their family caregivers on an individual and group basis
Coordinates with interdisciplinary team members to carry out and monitor resident care plan pertaining to social work support
Offers instrumental help to assist the client to resolve functional problems of daily living
Provides crisis intervention and short term assistance in addition to maintaining a caseload assignment
In consultation with the Executive Director, develops and implements policies pertaining to social work services
Participates in and contributes to interdisciplinary team conference of the LTC Home
Works closely with the Executive Director in providing social work services to residents of the LTC Home
Actively participates and contributes to the overall LTC leadership team meetings and functions
Keeps up-to-date with developments and best practices in the field
Other duties as assigned
Qualifications:
Bachelor degree in Social Work, or equivalent, and Registered Social Worker designation in Ontario
Minimum 2 years experience in counseling or group work, preferably in a long term care environment
Proficiency in computer applications
Ability to work independently, be self-directed to problem solve, exercising good judgment, tact and diplomacy
Empathetic attitude and desire to contribute towards the wellbeing of the elderly
Excellent communication and interpersonal skills
Satisfactory Police Vulnerable Sector Screen result
Ability to converse in Chinese.
