In the GAIN Community Team, the Occupational Therapist uses clinical judgment and standardized tools for assessing older adults. The position involves collaborating with other healthcare professionals to develop tailored care plans and improve daily living activities for seniors. Your expertise in therapy will guide seniors toward better health and wellness outcomes.
Key Responsibilities:
• Engage in comprehensive geriatric assessments
• Execute triage for new referrals to the program
• Collect and record relevant health data
• Provide clinical care aimed at maintaining daily activities
• Analyze frail seniors’ capabilities through interviews and assessments
Requirements:
• University degree in Occupational Therapy
• Registration with the Ontario College of Occupational Therapists
• Minimum two years of experience in geriatric care
• Familiarity with dementia and neurological conditions
• Mandatory driver's license for home visitations
