Under the direction of the Community Team Manager, the RPN will provide nursing services that include medication management, client assessments, and the development of tailored treatment plans. Your role will be pivotal in connecting clients with resources while delivering compassionate care. Key Responsibilities:
• Manage service coordination for individual treatment teams
• Conduct comprehensive assessments and treatment planning

Requirements:
• Registered Practical Nursing Diploma and certification
• One year in outpatient psychiatric care
• Preferably three years in inpatient settings
• Valid Ontario G Class driver’s license
• Access to transportation regularly
