In this full time position with the Seniors Mental Health Behavioral Support Services team, you will leverage your skills to provide essential support to Long Term Care, Acute Care, and community healthcare environments. Your expertise in mental health, behavioral health assessment, and education will be instrumental in facilitating knowledge networks and improving person-centred care models. You will engage with various healthcare stakeholders to ensure the highest standard of care for seniors facing complex needs.

Key Responsibilities:
• Provide educational consultations to healthcare teams
• Build effective relationships with community partners




• Collaborate on identifying community training requirements
• Engage as a member of the SMH BSS team
• Support the implementation of client-focused care models

Requirements:
• Health Care Discipline degree; Master’s preferred
• At least three years in community healthcare settings
• Knowledge in seniors’ behavioral health and intervention strategies
• Experience working within the PIECES framework
• Proven skills in crafting educational interventions
