Community Healthcaring KW provides comprehensive primary health care, including medical and dental services, health promotion, illness prevention, and community outreach. Through an interdisciplinary team of health care professionals, we deliver person-centred care that improves health equity, reduces barriers to accessing services, and addresses the social determinants of health to support the well-being of individuals, families, and communities across Waterloo Region.

We are currently recruiting for Registered Dietitian to provide services to Grand River Family Health Team. While these positions are employed by Community Healthcaring Kitchener-Waterloo, the Registered Dietitians' day-to-day work location (across 8 locations), schedule of hours, as well as applicable policies and procedures, will be determined by Grand River Family Health Team to which they are assigned.

Job Title: Registered Dietitian

Pay: $70,833.98 to $85,342.85 per year

Permanent, Full time 37.5h/week

Job Summary

The Registered Dietitian provides evidence-based nutrition assessment, counselling, and education to support the prevention and management of chronic. Working collaboratively with physicians, nurse practitioners, nurses, and other health care professionals, the Dietitian helps patients and families improve health outcomes, develop healthy lifestyle habits, and overcome barriers to healthy eating through individualized and group-based care.

Responsibilities

1. Provide individual education on nutrition and health assessments.
2. Develop client-focused nutritional plans.
3. Conduct group education sessions focused on nutrition.
4. Offer counseling and teaching tailored to clients' needs.
5. Support behavior changes within the scope of practice defined by the College of Dietitians of Ontario.
6. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
7. Use EMR and other relevant technology appropriately to support
8.



Work collaboratively to perform community nutrition needs assessments and identify barriers to acquiring adequate food security and facilitate and support community initiatives to improve food
9. Maintain accurate and up-to-date clinical notes in accordance with College guidelines and policy.
10. Work during both regular and extended hours (evenings/weekends) and in locations within Kitchener to be determined by Health Centre & Grandriver Health Team. An evening may be required.
11. Maintain a valid driver's license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with policy.
12. Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational
13. Use interpreters/translators as needed and effectively.
14. Collaborate with other team members to de-escalate situations or support clients while in crisis especially in the moments until added support is available.
15. Work individually and collaboratively in the development and implementation of quality improvement initiatives and
16. Work in a manner that preserves privacy and
17. Work in a manner that ensures client safety and minimizes risk to clients, volunteers.
18. Work in a manner that demonstrates self-reflection and personal accountability for work performance.
19. Adhere to all Health Centre and Grand River Health Team policies.
20. Adhere to appropriate infection control practices.
21. Perform any other duties as assigned by your supervisor.

Qualifications

1. Undergraduate degree in foods and nutrition from a recognized university or
2. Dietitians of Canada accredited Dietetic Internship or equivalent approved experience or graduate degree in community
3.



Registered member in good standing with the College of Dietitians of
4. Experience in diabetes education and care, preferably having achieved and then maintain the Certified Diabetes Educator
5. Comfortable counselling around a wide range of health problems common to family medicine. Some examples would include weight loss/gain, iron and vitamin deficiencies, IBS/constipation, Celiac disease, inflammatory bowel disease, eating disorders etc.
6. Demonstrated knowledge of clinical/nutrition guidelines appropriate for clients.
7. Three years nutritional counselling experience.
8. Experience in program development, implementation, monitoring and
9. Proficiency in the use of a personal computer and software programs including Microsoft Office, email and
10. Demonstrated ability to communicate effectively in English, both written and verbally. Second language is an
11. Demonstrated ability to work
12. Experience working effectively with culturally and economically diverse
13. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
