As a key member of the interdisciplinary team, the Social Worker will provide services to adult outpatients in the Orthopedic Fracture Clinic. The services provided range from psycho-social assessment and treatment; crisis intervention; advocacy; supportive counseling; consultation, patient/family/staff education; linking with community resources.
The Social Worker will enjoy a challenging practice, which requires excellent clinical skills, flexibility and a desire to work both independently and collaboratively in a variety of settings. This is a temporary, study-based position created to support a pilot trial of embedded social work services in the outpatient fracture clinic.
Qualifications 1. Masters degree in Social Work from an accredited University 2. Certification as a Social Worker with the Ontario College of Social Workers and Social Service Workers 3.



Minimum of 3 to 5 years of clinical social work practice involving individual and family counseling and therapy 4.
Previous experience in the provision of psycho-educational or therapy groups 5. Demonstrated expertise in interviewing, the completion of comprehensive psychosocial assessments and the development of a plan of action 6. Excellent communication, interpersonal, teamwork and crisis intervention skills 7.

Demonstrated skill and ability in provision of health qualified consultation and education 8. Demonstrated ability to identify and work with appropriate resources to address patient and family needs 9. Familiarity with the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act and Child and Family Services Act
