You will be responsible for providing a full range of social work services to a diverse patient population in a hospital setting, both acute and post-acute care. As a member of an inter-qualified team, you will provide comprehensive social work services in a high volume, fast-paced environment, including working collaboratively with physicians, community partners, hospital staff, patients and families. Additionally, you will provide high-quality counseling that addresses the social, emotional and economic aspects of illness, disability and recovery. You will assist patients and families in making important and appropriate decisions related to the impact of their illness and their discharge destination. You will advocate on behalf of patients and families and will liaise with community agencies regarding service provision, program initiatives and health care concerns. WHAT YOU WILL BE DOING:

You will be responsible for providing a full range of social work services to a diverse patient population in a hospital setting, both acute and post-acute care. You will work collaboratively within a multidisciplinary team of clinicians and physicians and develop strong partnerships with other community health services. Supporting outpatients therapeutically will also be required to help maintain and bridge supports pre and post inpatient and ER treatment. WHAT YOU MUST HAVE:





Regulated Health Professional who holds current active membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) Minimum Three (3) years recent experience working on a Schedule 1 mental health inpatient unit; Ability to establish a therapeutic relationship with Inpatients and outpatients and their supports experiencing mental health and addiction challenges within the context of individual and group therapy Ability to provide support using evidence-based treatment modalities such as Crisis Intervention, Interpersonal Therapy, Cognitive Behavioural Therapy, Solution-focused Therapy, and Motivational Interviewing to effect positive change in the individual’s mental state and coping Demonstrated clinical leadership skills with a proven track record of achieving results and sustained outcomes Advanced knowledge and skills in completing psychosocial screening, assessments and consultation Superior interpersonal and communication skills to effectively work with peers, outpatients, families, and community partners Advanced ability to promote the values of interdisciplinary team collaboration Ability to demonstrate a facilitative teaching approach with outpatients, families, staff and community partners,



and a good working knowledge of adult learning principles Excellent problem-solving, critical thinking and communication skills (both verbal and written) Computer literacy with proficiency in Microsoft Office Suite Effective communication and interpersonal skills with ability to positively influence others and develop trusting internal and external relationships Physical ability to carry out the essential duties of the position Ability to regularly attend work in accordance with the Attendance Support Program Acceptable performance record that is aligned with STEGH values and code of conduct HOURS:

The successful candidate must be available to work all tours applicable to the department. In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the department and be able to commit to the non-availability standards outlined in the Collective Agreement.
