The Program Facilitator(s) provides assessment, treatment planning and implementation for participants of the program. Delivers psycho-educational workshops and teaches mindfulness based relapse prevention and coping strategies. Utilizes community resources to facilitate achievement of treatment goals and transition planning. The position aids the participant in identifying and understanding personal issues and in establishing and achieving specific,
