We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our energetic healthcare team. In this vital role, you will provide high-quality patient care in a diverse setting, including assisted living, independent and a memory care neighbourhood. Your expertise will help improve patient outcomes, promote comfort, and support recovery through skilled nursing interventions. Duties
Deliver comprehensive patient care by assessing needs, developing care plans, and implementing appropriate nursing interventions.
Administer medications accurately and safely, following established protocols and medical orders.
Monitor patient conditions closely, documenting vital signs, symptoms, and responses to treatments using electronic health record systems such as MedECare and Yardi.
Assist with specialized procedures including laboratory specimen collection.
Collaborate with multidisciplinary teams to coordinate case management and ensure seamless continuity of care for patients with complex needs.
Maintain a safe setting by adhering to infection control standards and safety regulations in all settings.
Duties assigned by the Director of Wellness
Requirements
Valid registration as a Practical Nurse with the appropriate licensing body.
Proven experience in LTC home settings, or retirement homes.
Knowledge of medical terminology and medication administration procedures.
Experience working with diverse populations including seniors, and individuals with dementia.
Familiarity with electronic health records such as Yardi or MedECare is highly desirable.
Ability to work independently and collaboratively within a multidisciplinary team
