Cota seeks a dedicated Case Manager, Addictions Specialist for its Housing First initiative, focused on encampment and street homelessness in East Toronto. This full time position emphasizes recovery-oriented support and client empowerment.

In this role, you'll become part of the Assertive Housing First Team, working closely with 40 individuals in encampments. Candidates should have a relevant educational background and experience in addiction support or mental health. You'll provide vital case management, crisis intervention, and community resource linkage.

Key Responsibilities:



• Coordinate screening interviews and eligibility determinations • Build supportive relationships with clients • Complete assessments of client goals and needs • Guide clients in developing Client Safety and Recovery Plans • Lead group activities for skill development

Requirements: • Post-secondary degree/diploma and 1-3 years relevant experience • In-depth knowledge of recovery-oriented services • Advanced crisis management skills • Proficient in using databases and documentation systems • Must have a vehicle for client visits
