This energetic opportunity spans three transitional care units in Oshawa and Pickering, working directly under a Licensed Social Worker. Responsibilities include wellness checks, engaging community services, and providing educational resources. You'll also help arrange transportation for patient appointments, ensuring they receive holistic care.
Key Responsibilities:
• Execute care plans under professional supervision
• Provide support through virtual and in-person services
• Follow up with community services on referrals
• Organize transportation for patient access to services
• Efficiently manage active caseload priorities
Requirements:
• Diploma in Social Service Work is required
• Must be a registered member of the Ontario College
• At least 2 years of experience in community-based therapy
• Robust computer skills and internet access necessary
• Valid driver's license with access to a vehicle needed
