Community Living Hamilton is looking for a facilitator to support clients for about 12 months. Candidates should have a DSW or similar degree and two years of relevant experience. It’s essential to create Person Directed Plans while ensuring clients' dignity and safety.
Key Responsibilities:
• Facilitate engaging community activities for clients
• Create individualized plans focusing on personal development
• Foster a respectful and supportive relationship with clients
• Ensure a protected and secure environment for clients
• Advocate and assist clients in achieving their goals
Requirements:
• Degree or diploma in DSW, SSW, or Social Sciences
• Minimum two years' experience in similar environments
• NVCI and First Aid/CPR certifications
• Valid driver’s license for offsite activities
• Experience with diverse populations preferred
