Key Responsibilities:
• Serve individuals at risk of homelessness with empathy • Assess housing needs through structured tools • Connect clients to community resources and supports • Track service delivery and client records using HIFIS • Support emergency housing admissions when necessary
Requirements: • Two-year diploma in a relevant discipline • Minimum two years of social service experience • Additional training in mental health or addictions preferred • Ontario Driver’s Licence and reliable vehicle • Strong knowledge of homelessness risk factors
