PACE Independent Living is hiring a Community Resource Navigator in Ontario. The role works with clients, caregivers, and partners to advance independence, navigate health and social services, and coordinate service planning across multiple sites and client homes. It requires travel and some on-call hours.
The position is full time (40 hours/week), with core hours 8:30am–4:30pm, a staggered start option, and regular travel between PACE locations. A comprehensive perks package is included.
