Definition of work:

A Public Health Registered Nurse I/Community Health Registered Nurse I (PHNI/CHNI) combines knowledge from primary health care (including the determinants of health), nursing science, and social sciences in the assessment, planning, implementation, and evaluation of nursing care and the coordination of continuing care services in the above area. The PHNI/CHNI focuses on prevention, health restoration, maintenance or palliation in the delivery of care/coordination of service to clients, designated care givers or families. The PHNI/CHNI will ensure person and family centered care that wraps available resources around the individual wherever the individual resides. The PHNI/CHNI will establish a preventative, integrated, person and family centered approach driven by individualized assessment of needs and individual choice to promote aging in place. The PHNI/CHNI will practice utilizing the Home First philosophy in identifying, assessing and supporting individuals with complex care needs to be supported in their own home. Work is performed in accordance with the Canadian Community Health Nursing Standards of Practice and within the policies and procedures of the Central Regional Health Authority.



The PHNI/CHNI performs skilled community health nursing work carrying out community health programs in homes, schools, clinics, and other community settings. The successful candidate provides nursing care, assists in the assessment of community health needs, undertakes case management, and conducts school health programs and the provincial immunization program. The PHNI/CHNI is involved in health promotion activities, engages in community development, facilitates group sessions, conducts health education sessions in schools and communities, provides leadership in emergency preparedness and liaises with other community and professional personnel. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required qualifications:
ï‚· Undergraduate Degree in Nursing.
ï‚· Current registration with the Association of Registered
Nurses of Newfoundland and Labrador.
ï‚· Three (3) years clinical experience.
ï‚· One (1) year experience within the last three (3) years providing and coordinating community health nursing service.




ï‚· Current BLS certification for health care providers from the Heart and Stroke Foundation.
ï‚· Current Health Assessment certification or willingness to complete the next available course.
ï‚· Demonstrated exceptional leadership qualities and problem-solving skills, as well as, oral and written communication skills.
ï‚· Demonstrated ability to work well independently and collaboratively with others in an interdisciplinary team environment and community.
ï‚· Demonstrated ability to be self-directed and exercise independent nursing judgment in a community-based environment with limited supervision.
ï‚· Demonstrated initiative.
ï‚· Demonstrated ability to develop, implement, and evaluate community health nursing programs and healthy public policy in a population health framework.
ï‚· Demonstrated ability to apply basic computer skills.
ï‚· Willing to work a flexible work schedule.
 A valid Newfoundland and Labrador Class IV Driver’s License and the use of a private vehicle are conditions of employment.

ï‚· A satisfactory record of work performance and attendance is required for this position.
ï‚· A satisfactory certificate of conduct from the RCMP or local police authority.

Preferred qualifications:
ï‚· Demonstrated skill in using health care-based computer systems (CRMS, Meditech).
ï‚· CNA certification in Community Health Nursing.
ï‚· Completion of the CNA Community Health Nurse Certification program.
