Key Responsibilities: • Perform client intake assessments in home and community environments • Assess activities of daily living (ADLs)
effectively • Evaluate safety and mobility needs of clients • Develop assessment documentation and recommendations • Collaborate with families and healthcare stakeholders
Requirements: • Current registration with COTO • At least 1 year of relevant assessment experience • Excellent communication and interpersonal skills • Reliable transportation and driver's licence preferred • Ability to obtain professional liability insurance
