Make a Difference in Rural Primary Care
At Bighorn Primary Care Network, we're committed to improving access to high-quality healthcare for individuals and families across our communities. We are seeking a compassionate and skilled Family Practice Nurse to join our collaborative healthcare team at the Hinton Medical Clinic.
In this role, you'll practice to your full scope alongside family physicians and interdisciplinary healthcare professionals, helping patients achieve better health outcomes through prevention, chronic disease management, health education, and coordinated care.

What You'll Do
As a key member of our primary care team, you will:
Ø Deliver patient-centered care in partnership with physicians and PCN team members.
Ø Perform health assessments, develop care plans, and provide ongoing follow-up and support.
Ø Support patients living with chronic conditions, including diabetes and other complex health needs.
Ø Assist with mental health and addictions screening and connect patients to appropriate resources.
Ø Review laboratory results, support medication management, and provide patient and family education.
Ø Provide clinical services such as wound care, injections, immunizations, and other nursing interventions.
Ø Coordinate care transitions and help patients access community and healthcare resources.
Ø Maintain accurate, timely documentation in the electronic medical record (EMR).
Ø Contribute to quality improvement initiatives that enhance patient outcomes and clinic operations.
What You BringRequired Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed and in good standing to practice in Alberta.
- Current BLS/CPR certification.
- Criminal Record Check and Vulnerable Sector Check.
- Strong communication, assessment, and clinical decision-making skills.
- Ability to work collaboratively within an interdisciplinary healthcare team.
- Proficiency with EMR systems and standard workplace technology.
- Valid Alberta driver's license and access to a reliable vehicle insured for work-related travel.
Preferred Qualifications
- Minimum three years of experience supporting patients with chronic disease management.
- Experience in primary care, family practice, community health, or team-based care settings.
- Knowledge of the Patient Medical Home model and the social determinants of health.
- Experience working in a rural healthcare environment.

Pay: $34.85-$55.73 per hour
