In this position, you will be responsible for patient assessments and the coordination of services to facilitate safe transitions from hospital to home. We seek experienced healthcare professionals—such as nurses, physiotherapists, and social workers—who can adapt to changing patient needs while delivering integrated care. Your role will be crucial in supporting both complex and non-complex cases.
Key Responsibilities:
• Conduct patient assessments and establish trusting relationships
• Design service plans that reflect client needs and preferences
• Collaborate with healthcare teams and community resources
• Implement and monitor care plans in accordance with quality standards
• Facilitate discharge planning and connect clients to alternative services
Requirements:
• Degree in nursing, physiotherapy, social work, or similar
• At least 2 years of related skilled experience
• Knowledge of community health services in Champlain region
• Must be fully vaccinated against COVID-19
• Recent Vulnerable Sector Check required
