In this Full time position, you will work alongside Registered Nurses and the Clinical Lead to coordinate care for residents, addressing their health and wellness needs. This role emphasizes person-centered care and requires a high level of resilience in challenging situations. You will directly impact individuals who have faced marginalization and thrive in an inclusive and diverse workplace.
Key Responsibilities:
• Liaise with pharmacy and community resources for medication management • Conduct health assessments and develop supportive care plans • Maintain communication with primary care providers • Build therapeutic relationships with residents through trust • Lead and participate in interdisciplinary team meetings
Requirements: • Registered Practical Nursing program completion • Current registration with College of Nurses of Ontario • Commitment to cultural safety and anti-oppressive practices • Experience with mental health services is an asset • A valid driver’s license and access to a vehicle
