Child and Youth Worker Status: Regular Part Time Mental Health & Addictions 4 Hour Shifts, days Schedule (Subject to Change) Location: Off site: Wage Schedule: $36.400 - $42.180 per hour Posting Reference: (phone hidden) Lakeridge Health is more than a hospital. Working as part of a current collaboration between the Child, Youth and Family Program and the Durham Catholic School Board, the successful candidate's primary duties will be to: support youth participating in PATH: Partnerships for Academic and Therapeutic Healing .

You will work with patients, families and the team to develop individualized care plans that address the mental health concerns impacting individual functioning and work toward a goal of school reintegration. and initiate and/or participate in discharge planning activities, including community agencies/other hospital programs as appropriate and involving community visits as necessary. Ability to assess changes in youth presentation/mental status, behaviour, and risk is critical, as well as the ability to develop, coordinate and implement a plan for safety. As PATH programming runs September through June yearly, for the months of July and August, the successful candidate will be assigned to Central Connect, supporting intakes and navigation throughout the mental health program.

Connect hours are flexible and will be determined based on need, equivalent to 0.5 hours per pay. College diploma/university degree in child and youth work or equivalent combination of education and experience

- Extensive experience in crisis assessment and inpatient or educational setting hospital based treatment of children and adolescents experiencing severe mental health disorders;

Experience and training in evidence based treatment modalities such as CBT and DBT

- Demonstrated knowledge of health care facilities, community support systems and social service agencies
- Demonstrated strong interpersonal skills and effective written and verbal communication skills and the ability to foster good public relations and effective working relationships in a family centered environment
- Must have satisfactory performance record
