Overview

Just Like Family Home Care is looking for a passionate and experienced Client Care Coordinator to join our growing team in Surrey and the surrounding communities. In this dynamic role, you’ll build meaningful relationships with clients, families, caregivers, and community stakeholders, while helping to ensure exceptional, personalized care. Duties

- Build professional relationships with community organizations, senior-serving professionals, retirement communities, assisted living facilities, healthcare providers, physiotherapy clinics, occupational therapists, social workers, case managers, and other referral partners to generate client referrals.
- Target potential referral partners, conduct outreach through in-person meetings and business to business visits to increase awareness of our services to the community.
- Attend trade shows, networking opportunities and community events to promote the company and increase brand awareness.
- Manage an assigned case load of clients and maintain regular responsive communication with clients and their families.
- Provide ongoing support to clients and families by addressing questions and concerns, coordinating changes in care needs or schedules and ensure that all relevant information is communicated accurately and promptly.
- Collaborate with management to improve operational processes, client experience and business growth.




- Participate in after hours support or emergency coordination when required.

Requirements

- Minimum of 2 years experience in home care, community health, healthcare coordination or a related field.
- Solid knowledge of home care operations, BC’s medical system and client care planning.
- Excellent communication, interpersonal, customer service and relationship building skills.
- Ability to work independently while managing multiple priorities.
- Confidence to make multiple phone calls and have difficult conversations with families.
- Proficiency with Google Workspace, CRM software or scheduling software.
- Valid BC Driver's Licence and access to a reliable vehicle.
- Ability and willingness to travel throughout White Rock, Surrey and Langley.
- Ability to successfully complete a Criminal Record Check - Vulnerable Sector
- Legally authorized to work in Canada.

Pay: $50,000.00-$56,000.00 per year
