Site: West Park Healthcare Centre Collaborative Academic Practice Salary: $65.09 - $76.14 per hour (start rate is non-negotiable and in accordance with collective agreement requirements) Shifts: Weekdays Temporary Full Time (until January 2027) The Nurse Practitioner (NP) contributes to quality patient care, and improvements in organization and system outcomes. The NP demonstrates advanced knowledge, skill and judgment in providing care to patients that is consistent with the College of Nurses of Ontario's Practice Standards for Nurse Practitioners and the Canadian Nurses Association Competencies for Nurse Practitioners. The NP works autonomously and collaboratively with physicians, fellow Nurse Practitioners, and the interprofessional team within the Complex Continuing Care program to provide clinical care to patients and to develop, implement and evaluate evidence informed practices within the Complex Continuing Care patient population at West Park Healthcare Centre.
Identifies and orders diagnostics tests/procedures in accordance with the Primary Health Care Nurse Practitioner scope of practice, approved medical directives, and the Public Hospital Act Initiates and manages the care of patients by formulating and implementing a care plan to treat disease or disorders, within scope of practices, including prescribing pharmacological, complementary or counselling interventions; Plans and implements strategies to promote health and prevent illness with individuals,
families and groups Initiates individual, family and group promotion of health/wellness Provides individual counseling/instruction to facilitate patient/family understanding and management of specific health conditions and impact of chronic disease. Participates in quality management initiatives and decision-making process on issues that impact clinical practice Order and interpret screening and diagnostic tests, make appropriate referrals to internal and external health care professionals, prescribe medications, and perform procedures within the NP’s legislated scope of practice and the Centre’s policies, including admitting and discharging patients as well as transferring patients to acute care. Communicate assessment findings, diagnoses, and recommendations for further testing or referrals to patients.
Act as a preceptor, resource, consultant and coach to nursing colleagues, members of the interprofessional team and students. Guide, initiate and provide leadership in the development and implementation of policies, standards, practice guidelines, quality assurance, education and research initiatives, both corporately and program/unit specific. Maintain eligibility to be cross appointed to an academic institution and belonging to the Nurse Practitioner Association of Ontario and other relevant organizations Embed research-based knowledge into daily practice and to support and educate other members of the health care team, including students.
Participate in the development and delivery of educational approaches for both patients and practitioners, including students. Collaborate with nurses, physician colleagues and the interprofessional team to evaluate outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends of specific patient populations.
Recommend and participate in quality improvement opportunities to promote protected, timely, effective, efficient, equitable patient-centered care.
Participate in research that contributes to improved outcomes for patients Completion of a recognized Masters degree program in Nursing Nurse Practitioner- Adult (NP-Adult), or Nurse Practitioner- Primary Health Care (NP-PHC) Current certificate of (licensure)/registration in good standing with the College of Nurses of Ontario as RN (EC) Current certification in Basic Cardiac Life Support, ACLS an asset Three (3) years of Nursing experience in the extended class preferred Nursing experience in respiratory care and care of patients who require artificial airway or chronic non-invasive ventilatory needs Experience in nursing research methodology Ability and commitment to develop a collaborative practice with medical, allied health and alternative health practitioners Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards Knowledge of applicable legislative, UHN and/or departmental policies Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand Excellent customer service skills In addition to working alongside some of the most talented and inspiring healthcare professionals in the world, UHN offers a wide range of benefits, programs and perks.
