Community Health Nurse The Tsartlip First Nation is pleased to invite applicants to apply to the permanent, full-time Community Health Nurse (CHN) position.

The Community Health Nurse provides a comprehensive range of nursing functions and services in the Tsartlip First Nation community in the areas of health promotion, disease prevention, education, and assessment. The Nurse works within the context of a culturally secure, holistic, client-centered focus. The CHN reports to the Health Programs Manager.

The wage range for this position is $42.25 - $57.03 per hour.

Education and Experience

- Bachelor’s degree in nursing from a recognized University;
- Current registration with BC College of Nurses and Midwives as a practicing RN registrant
- Minimum 1 year of recent experience in Public Health/Home Care Nursing;
- Experience working within a First Nations community an asset;
- Valid First Aid and CPR Level C certificates required;
- Valid BC Class 5 driver’s license and reliable vehicle plus a clean driver’s abstract is required;
- Clean criminal record check with Vulnerable Sector required.

Duties And Responsibilities

- Provides care and service that meets the standards for Registered Nurses within the BC College of Nursing Professionals (BCCNP).



Works within the scope of practice for Registered Nurses as determined by relevant legislation (Health Protections Act), BCCNP, organizational policy and own clinical competency.
- Seeks guidance from BCCNP and/or FNHA Regional practice support when required;
- Utilizes evidence and research to influence health policies and programs. Practices and synthesizes nursing and health knowledge from a broad range of theories, models, and frameworks to address the health status of populations, inequities in health, determinants of health, strategies for health promotion,disease and injury prevention, health protection and case management.
- Applies and utilizes nursing knowledge and critical thinking to choose options and to plan, implement and evaluate interventions for clients with acute, chronic or palliative health care needs and their caregivers in the client’s home and/or community setting. Develops client centered care plans and works with clients to develop short- and long-term goals for health and wellbeing;
- Provides care, delivers programs and acts as an advocate for services that promote and protect the health and wellbeing of individuals, family groups and communities;




- Provides health promotion, disease prevention, education, assessment, and consultation to individuals, groups, and community;
- Immunizes infants, children, and adults against vaccine preventable diseases; Educates clients on the purpose and outcomes of immunization;
- Involves individuals, families, groups, and communities as active partners to take action to address health inequities and foster a self-management care approach to health care;
- Involves individuals, families, groups, and communities as active partners to take action to address health inequities and foster a self-management care approach to health care with an emphasis on health living,illness prevention, health education, chronic disease management, and clinical intervention;
- Advocates for, and uses culturally relevant and appropriate approaches, when building relationships and providing nursing services;
- Coordinates the FNHA core programs to meet the needs of the clients through promoting family and community support while encouraging client’s independence. Invites individuals, families, groups, and communities to be involved as active partners to foster a self-management care approach for chronic conditions;
- Participates in collaborative, interdisciplinary and inter-sectoral partnerships to enhance the health of individuals, family groups and populations.
