As a Case Manager, you will manage a caseload of clients experiencing severe mental illness in the Mississauga/South Etobicoke area. Your responsibilities include providing recovery-oriented support, conducting comprehensive assessments, and engaging individuals throughout their recovery process. Collaborating closely with treatment teams and clients, you will develop individualized care plans and support their journey towards independent living.

Requirements: • Bachelor’s degree or Community College diploma in health-related field • Minimum five years' experience with clients experiencing severe mental illness • Knowledge of recovery-oriented and trauma-informed care • Robust organizational and communication skills • Valid driver’s license and access to a vehicle
