In this position, you will guide and support children and youth with complex care requirements. Your responsibilities include navigating them through healthcare and social services while promoting best practices in Indigenous health. You will also lead workshops and work closely with community health staff to amplify their capabilities.
Key Responsibilities:
• Guide families through complex health service systems
• Promote collaboration among health service providers
• Conduct workshops on mental health and wellness
• Maintain data reporting and client referrals
• Assist in designing individualized health plans
Requirements:
• Bachelor’s Degree in a related health discipline
• At least 3 years of relevant experience in BC
• Proven understanding of First Nations health models
• Experience in case management and coordination
• Proficient with Microsoft Office applications
