Job Summary

We are seeking a compassionate and dedicated Home Care Worker to provide exceptional in-home support to one individual in need of assistance with daily activities. This vital role involves delivering personalized care that promotes independence, safety, and well-being. As a Home Care Worker, you will be part of a small team that are working together to play a key part in enhancing the quality of life by offering attentive, respectful, and professional care tailored to nique needs.

Your energetic approach and commitment to caregiving will help create a positive environment where the client feel valued and supported every day. You will work closely with the primary care-giver and family to support a home based dementia journey.

Duties

- Assist clients with activities of daily living, including bathing, dressing, grooming, and mobility support
- Provide patient care by monitoring health status, administering medications as prescribed, and reporting any changes to the primary care-giver.
- Support clients with meal preparation, ensuring nutritious and balanced diets tailored to dietary restrictions or preferences
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment
- Offer companionship and emotional support to reduce feelings of loneliness or isolation
- Be prepared in the future to assist with mobility and transfer techniques,



including heavy lifting when necessary, using proper safety procedures
- Document care activities accurately and communicate effectively with family members and healthcare teams to ensure continuity of care

Qualifications

- Prior experience in assisted living facilities, nursing homes, hospice care or home care environments is highly desirable
- Solid background in patient care, senior care, dementia care or working with individuals with disabilities
- Knowledge of medication administration and basic nursing procedures
- Ability to perform heavy lifting safely and effectively when assisting clients with mobility needs
- Excellent communication skills with a compassionate attitude towards clients and their families
- Reliable, energetic, and committed to providing high-quality caregiving services
- Certifications related to caregiving or healthcare are a plus but not mandatory; willingness to obtain necessary training is expected
