Housing Stability Case Manager

Position Summary The Housing Stability Case Manager plays a key role in supporting individuals who are unhoused or precariously housed to secure and maintain stable housing. Using a trauma-informed, client-centered approach, this position focuses on preventing housing loss, navigating barriers to housing, and walking alongside clients as they transition into safe, sustainable living situations.
This role requires a high level of collaboration with internal staff and community partners, as well as robust advocacy and practical problem-solving skills.
Key Responsibilities
Client Support & Case Management
- Build trusting relationships with clients (“guests”) through compassionate
- listening and consistent engagement
- Conduct needs assessments focused on housing stability, risks of eviction, and barriers to securing housing
- Develop individualized housing plans with clear, measurable goals
- Provide ongoing case management to support housing retention and stability
Housing Navigation & Advocacy
- Assist clients in searching for appropriate housing listings (online and local networks)
- Make phone calls and advocate on behalf of clients with landlords, property managers, and housing providers. Arrange and, when needed, accompany clients to viewings and appointments
- Support clients in completing rental applications and securing necessary documentation
- Coordinate transportation to housing-related appointments when required
Prevention & Stabilization
- Identify early warning signs of housing instability and intervene proactively
- Work to prevent eviction through mediation, resource connection, and advocacy
- Connect clients with financial assistance programs, rent banks, emergency supports, and employment resources
Resource Coordination
- Connect clients to relevant community supports including shelters, supportive housing, mental health services, income supports, legal aid, employment and other training & skills based learning
- Collaborate closely with the Wraparound Care Coordinator & Case Management Team to ensure holistic, wraparound support
- Assist clients in transitioning into housing, ensuring basic needs are met for successful tenancy
Collaboration & Documentation
- Participate in team meetings, case reviews, and collaborative care planning
- Maintain accurate, timely, and confidential client records in compliance with organizational policies and applicable privacy legislation (e.g., PHIPA/PIPEDA) in Charity Tracker and HIFIS
- Track outcomes related to housing placements and stability for reporting and funding purposes
Qualifications Education & Experience
- Diploma or degree in Social Work, or Social Service Work
- Minimum 2 years of experience working with vulnerable populations, particularly individuals experiencing homelessness or housing instability
- Experience in housing navigation, case management, or community outreach is strongly preferred

Additional Assets
- Lived experience of homelessness or poverty (valued and considered an asset)
- Understanding of mental health and substance use challenges
- Valid driver’s license and access to a vehicle (if required)
- First Aid and CPR training
- Ability to work occasional evenings to accommodate client and landlord schedules
Working Conditions
- Combination of office, community-based work, and field visits●
- Regular interaction with individuals experiencing crisis, trauma, and complex life circumstances
- Flexible and responsive work environment requiring adaptability
- Hours: 8am-4pm, 1 split shift Friday/Saturday 10am-2pm a month
