Title: Intake and Client Navigator
Employment Terms: Regular Full-Time
Schedule: 35 hrs per week, with flexibility between 8 am and 8 pm on weekdays, and weekends as required by the HATP/TNO programs.
Salary: $56,547 per year

Location: Health Access Thorncliffe Park (HATP) – TPCH, 45 Overlea Blvd. (Unit 200), Toronto, ON M4H-1C3 and other locations
Background:
Health Access Thorncliffe Park (HATP) is committed to providing coordinated Health & Wellness services to the Thorncliffe Park community. It is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park. HATP is an cutting-edge partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the Ontario Health- Toronto region.
This position is part of the HATP collaboration, employed by TNO, and works within the Community Engagement Team.
TNO is a not-for-profit community based multicultural and multi-service agency, located in Toronto, Ontario, with a vision of neighbourhoods where everyone belongs and succeeds.
Position Summary:
The Intake & Client Navigator is part of the HATP Pediatric Clinic team and serves as the first point of contact for children, youth, and families accessing services. The role supports intake, referrals, care coordination, and family navigation, working closely with healthcare providers, schools, and community partners to ensure accessible, family-centred, and coordinated care.
Main Tasks and Responsibilities
· Serve as the first point of contact for intake, assessment, and service navigation.
· Coordinate referrals to healthcare providers, specialists, and community services.
· Facilitate communication between families, care providers, schools, and community agencies.
· Provide care coordination, case management, and support for complex client needs.
· Participate in interdisciplinary team meetings, family meetings, and care planning activities.
· Conduct outreach and education sessions for families and community partners.
· Maintain accurate documentation in the Electronic Medical Record (EMR) system.
· Support quality improvement initiatives, patient issue resolution, and clinic operations.
Skills and Qualifications
· Bachelor’s degree in health sciences, Social Sciences, Life Sciences, or equivalent combination of education and experience.
· 2-3 years of experience in healthcare, community services, or a related setting.
· Strong communication, interpersonal, organizational, and problem-solving skills.
· Experience with Microsoft Office and Electronic Medical Records (EMR), preferably PS Suite.
· Ability to work effectively in a fast-paced, interdisciplinary environment.
· Knowledge of care coordination, community resources, and family-centred service delivery.
· Commitment to confidentiality, equity, and client-centred care.
· Second language relevant to the community served is an asset.
