You will serve clients identified by health provider partners, ensuring they access essential programs that bolster their health and well-being. Your ability to assess needs and connect clients to resources will be vital in overcoming social barriers. This mobile position may require some evening and Saturday hours as you facilitate client services wherever needed.
Key Responsibilities:
• Connect clients to health and social resources
• Provide crisis intervention and brief counselling
• Assist in community outreach and training sessions




• Advocate for clients at service coordination committees
• Engage with community partners for resource sharing
Requirements:
• Bachelor of Social Work or equivalent degree
• Two years relevant experience in crisis intervention
• Solid communication skills in English, French preferred
• Valid driver’s license and access to a vehicle
• Maintain membership in professional college
