: Reporting to the manager of the COPD program, the nurse clinician: has primary clinical responsibility for the COPD program. Utilizes established criteria to assess and plan each client's self-management care needs, providing appropriate intervention to ensure timely access to care. Assesses client needs/concerns through chart review, history taking, physical assessments, and lab data and in consultation with physicians and multidisciplinary team members.
Liaises with multidisciplinary team members, hospital departments, and community agencies to advocate for continuity and coordination of care. Provides efficient and effective self-management education/counselling for clients/families/caregivers in all aspects of COPD management, care and treatment. Assists in the development, monitoring and evaluation of programs, protocols, guidelines and clinical standards of practice, in collaboration with the manager, physicians and multidisciplinary team members.

Experience
- Experience delivering applicable clinical programs/services
- Minimum of 1 year experience teaching/counseling, and leading group activities
- Experience in applicable policies and procedures
- Minimum of 1 year experience in patient/family/professional education required
- Experience in clinical research, program development and evaluation
- Experience in quality assurance initiatives
- Minimum of 1 year experience in program design, implementation and evaluation
- 5 years of clinical experience related to patients with COPD required
- 1 year experience in program/resource development
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Respiratory Trec Certificate
- Basic Life Support Certificate (BLS)
Other Education and Training
- Nursing diploma is acceptable
