- Perform client intake assessments in home and community environments
- Assess activities of daily living (ADLs) effectively
- Evaluate safety and mobility needs of clients
- Develop assessment documentation and recommendations
- Collaborate with families and healthcare stakeholders Requirements:
- Current registration with COTO
- At least 1 year of relevant assessment experience
- Excellent communication and interpersonal skills
- Reliable transportation and driver's licence preferred
