About The Peterborough Family Health Team
Peterborough Family Health Team (PFHT) is a collaborative primary care organization that works alongside family physicians and community partners to provide comprehensive, team-based care to the residents of Peterborough and Peterborough County. Our interdisciplinary team works together across multiple clinical sites.
PFHT supports clinical excellence, innovation, and digital health solutions that enhance patient care, improve provider workflows, and protect the privacy and security of health information.
Overview
Reporting to the Clinical Manager(s), the Clinical Pharmacist is an integral member of the primary care team, working collaboratively with physicians, nurse practitioners, and other healthcare professionals to optimize medication therapy, improve clinical outcomes, and enhance patient care. This position emphasizes a team-based approach to chronic disease management, medication reconciliation, patient education, and quality improvement initiatives in a primary care setting. This is a permanent, part-time position (26.25 hours/week) based out of Peterborough.
Responsibilities The Pharmacist provides direct patient care in collaboration with patients, physicians, nurse practitioners, and other members of the health care team.
Key responsibilities include
Collaborative Patient Care
- Participate in direct patient care activities, including comprehensive medication reviews and the development of individualized care plans.
- Monitor and manage chronic conditions such as hypertension, diabetes, hyperlipidemia, asthma, and anticoagulation therapy within scope of practice or through Medical Directives.
- Assess and provide treatment for minor ailments in alignment with clinical guidelines and scope of practice.
- Provide evidence-based recommendations on drug therapy, dosing, adjustments, and monitoring.
Medication Management
- Conduct medication reconciliation during care transitions (e.g., hospital discharge).
- Identify and resolve medication-related problems such as adverse reactions, drug duplications, or non-adherence.
- Assist patients in navigating drug access programs
- Collaborate in de-prescribing initiatives to optimize and rationalize medication use,
with particular emphasis on vulnerable populations.
Patient and Caregiver Education
- Provide education on safe and effective medication use, potential side effects, and adherence strategies.
- Support patients and caregivers with self-management strategies, including lifestyle modifications (e.g., diet, physical activity, smoking cessation).
- Deliver group and individual health education sessions to enhance chronic disease self-management.
Team Collaboration and Communication
- Participate in multidisciplinary team huddles, case conferences, and care planning meetings.
- Document clinical assessments, interventions, and recommendations in the electronic medical record (EMR).
- Serve as a drug information resource for physicians, nurse practitioners, and allied health professionals, ensuring timely, evidence-based, and unbiased guidance.
Career development and Leadership
- Stay current with evolving clinical guidelines, drug therapies, and healthcare regulations.
- Mentor pharmacy students or residents, as applicable.
- Contribute to program development within the Peterborough Family Health Team, with a focus on chronic disease management initiatives.
- Promote a culture of continuous quality improvement and interprofessional collaboration.
Other related duties may be assigned in the interest of the efficient operations of Peterborough’s Family Health Team and the effective management of our patient populations.
Hours Of Work
Monday to Friday – Hours of service are pre-established per employment contract and may require some evenings and weekends. Any deviation from the established schedule shall not negatively impact upon the provision of service to the patient population.
Qualifications
- Bachelor of Science in Pharmacy from a recognized university
- Registration with the Ontario College of Pharmacists (Part A)
- Two years’ experience preferred,
or community/hospital residency and/or Pharm D an asset: involvement in Primary Care preferred
- Additional training or credentialing in pharmacotherapy (eg.
Certified Diabetes
Educator, PHARM-MOAT – Management of Oral Anticoagulant Therapy Course from U of Waterloo, CAMH TEACH certification) will be necessary depending on the specific pharmacist’s role.
- Additional training or credentialing in pharmacotherapy (eg.
Certified Geriatric
Pharmacist, Certified Asthma Educator, Hypertension etc) considered an asset.
- Ability to work both independently and effectively in an interdisciplinary team environment
- Experience in providing patient-centred care and evidence-based unbiased drug information
- Proof of professional liability insurance
