As a vital team member under the Director of Primary Care, you will conduct health assessments, support chronic disease education, and triage urgent cases. This position involves full time hours of 77.50 biweekly, fostering a rewarding environment committed to health promotion. Your expertise will empower marginalized communities through targeted healthcare services.

Key Responsibilities:



• Perform sexual health examinations and comprehensive assessments • Educate patients on chronic disease management • Provide urgent triage and supportive care • Maintain ongoing case management for high-risk populations • Work collaboratively with healthcare professionals to enhance care

Requirements: • Baccalaureate in Nursing or equivalent clinical experience • Active registration with the College of Registered Nurses of Manitoba • Current Cardiopulmonary Resuscitation certification • Asset: RN Authorized Prescriber for reproductive health • A dedication to serving diverse community needs
