Brant Community Healthcare System (BCHS) is a 300+ bed acute care facility located 20 minutes west of Hamilton in the Grand River Valley. BCHS serves the Brantford and Brant County communities with specialty services such as surgical, rehabilitation, internal medicine subspecialties, and palliative care. The Department of Stroke, Rehabilitation, Complex, and Palliative Care is seeking family physicians to provide post‑acute hospitalist care at the Brantford General Hospital and Willet Hospital in Paris. Physicians will deliver medical‑rule‑based (MRP) care for patients admitted to the Rehabilitation, Complex Care units, and Integrated Stroke Unit (ISU). These patients are longer‑term, lower‑acuity cases managed in a hospital setting. The team offers 24/7 specialist support in internal medicine, ICU, and surgical services.
Responsibilities
- Provide MRP care for post‑acute patients in the Rehabilitation, Complex Care, and Integrated Stroke units.
- Collaborate with physicians, nursing staff,
and other clinicians to deliver holistic, patient‑centered care.
- Participate in medical education and service improvement initiatives.
- Liaise with community and hospital managers to ensure continuity of care.
Qualifications
- Independent practice license from the College of Physicians and Surgeons of Ontario.
- Certification with the College of Family Physicians of Canada or the Royal College of Physicians and Surgeons.
- Hospitalist experience considered an asset.
- Training or strong interest in stroke care is encouraged.
- Strong interpersonal and team-oriented skills, and a commitment to service excellence.
Compensation
Daily stipend plus fee‑for‑service remuneration.
