Unify Health is a physician-led multidisciplinary healthcare clinic providing a wide range of specialist and diagnostic services. Our team works closely with referring physicians, patients, specialists, and healthcare partners to ensure patients receive timely access to the care they need.

We are looking for a Referral Coordinator to join our growing team. This role is ideal for someone who is organized, detail-oriented, comfortable working in a fast-paced medical environment, and enjoys coordinating care from referral to appointment.

Position Summary The Referral Coordinator will be responsible for managing incoming referrals through multiple channels, including Ocean eReferral, Phelix inbound fax, and other referral sources. The role will ensure referrals are received, reviewed, accurately entered, appropriately triaged, and followed through to appointment booking.

The successful candidate will work closely with physicians, clinical assistants, administrative staff, referring offices, and the operations team to ensure referrals move through the clinic efficiently and patients receive timely access to care.

Key Responsibilities

- Receive, review, and process incoming referrals from physicians, hospitals, clinics, and other healthcare providers.
- Manage electronic referrals received through Ocean eReferral and ensure referrals are appropriately processed and routed.
- Monitor and manage incoming referrals and documents through Phelix, Unify Health’s inbound fax sorting and referral management platform.
- Review incoming faxes and electronic referrals to identify new referrals, supporting documentation, physician correspondence, and other patient-related documents.
- Review referrals for completeness and ensure required clinical information, investigations, and documentation are available.
- Enter and maintain accurate patient and referral information in Accuro EMR and other clinic systems.
- Triage and route referrals to the appropriate specialty, physician, or clinical team based on established clinic criteria.
- Follow up with referring offices when additional information, documentation, or investigations are required.
- Contact patients by phone, email, or other approved communication methods regarding their referrals and appointments.
- Schedule consultations, diagnostic testing, and follow-up appointments based on provider availability and referral requirements.
- Monitor referral queues, waitlists,



and outstanding referrals to ensure timely follow-up.
- Coordinate with physicians and clinical assistants regarding referral requirements, appointment urgency, and scheduling needs.
- Communicate professionally with referring physicians, specialist offices, hospital staff, patients, and other healthcare professionals.
- Follow up on referrals that have not been booked, confirmed, or completed.
- Ensure referrals are appropriately documented and closed once the patient has been scheduled or the referral has otherwise been resolved.
- Identify urgent or time-sensitive referrals and escalate them appropriately according to clinic procedures.
- Assist with referral-related phone calls and inquiries.
- Maintain accurate and organized referral documentation within the appropriate systems.
- Identify referral workflow issues and communicate them to the clinical operations team.
- Assist with improving referral workflows and identifying opportunities to reduce delays, duplicate referrals, and administrative errors.
- Maintain patient confidentiality and comply with applicable privacy and healthcare regulations.
- Support other administrative and clinical coordination duties as required.

Systems & Technology The Referral Coordinator will regularly work with multiple healthcare and administrative platforms, including:

- Accuro EMR – patient charting, referral documentation, appointment scheduling, and clinical records.
- Ocean eReferral – receiving, reviewing, processing, and managing electronic referrals.
- Phelix – sorting and managing inbound faxes and referral-related documents.
- Other internal communication, scheduling, and administrative systems used by Unify Health.

Experience with these systems is an asset; however, training may be provided to the right candidate who demonstrates solid healthcare administration experience and the ability to learn new systems quickly.

Qualifications

- Previous experience in a medical office, clinic, hospital, specialist office, or healthcare administration is preferred.
- Previous experience with referral coordination or specialist booking is strongly preferred.
- Experience with Accuro EMR is an asset.
- Experience with Ocean eReferral is an asset.




- Experience with Phelix or other inbound fax/referral management platforms is an asset.
- Strong understanding of medical terminology and healthcare workflows.
- Excellent organizational and time-management skills.
- Strong attention to detail and ability to identify missing or inconsistent information.
- Excellent verbal and written communication skills.
- Comfortable communicating with patients, physicians, and healthcare professionals.
- Ability to manage a high volume of referrals and multiple competing tasks while meeting deadlines.
- Ability to work independently while also collaborating effectively with a multidisciplinary team.
- Comfortable working in a fast-paced environment with changing priorities.
- Strong computer skills and ability to learn new systems quickly.
- Ability to maintain strict patient confidentiality and professionalism at all times.

What We Are Looking For We are looking for someone who is reliable, organized, proactive, and patient-focused.

The ideal candidate will take ownership of a referral from the time it is received until the appropriate next step is completed. This includes monitoring Ocean eReferrals, reviewing and sorting inbound faxes through Phelix, entering information into Accuro, following up for missing documentation, coordinating with providers, and ensuring patients are booked appropriately.

You should be comfortable managing a high volume of referrals while maintaining accuracy, efficiency, and attention to detail.

A strong candidate will be able to recognize when a referral requires clarification or escalation and will communicate promptly with the appropriate physician, referring office, or internal team member.
