What We’re Looking For:

We are seeking skilled and compassionate Registered Nurses (RNs) to join our team. Whether you are an experienced professional or a recent graduate, we welcome you to explore part-time or full-time opportunities with us. Join our mission to better support seniors in the comfort of their own homes.

Key Responsibilities:

- Conduct wellness nurse home visits, providing 30-40 minute appointments to support stable patients managing chronic diseases in maintaining their independence.
- Deliver exceptional nursing care to patients in line with care plans and best practices.
- Act as the eyes and ears of the doctor, focusing on preventative and treatment-based care.
- Monitor patient progress, and document care accurately.
- Perform assessments of cognitive, cardiovascular, respiratory, and other systems.
- Address Activities of Daily Living (ADLs), including social well-being, mental health, and sleep.
- Educate patients and their families to promote wellness and recovery.
- Coordinate care on behalf of the GP, including arranging diagnostics and labs.




- Provide services such as wound care, vaccinations, personal care, and thorough charting by exception.
- Adhere to regulatory guidelines and maintain the highest standards of patient safety and confidentiality.

Skills and Qualifications:

- Positive, can-do attitude with excellent relational and observational skills.
- Proficiency in checking vital signs and conducting comprehensive assessments.
- Strong time management and attention to detail.
- Pharmacological knowledge.
- Experience or knowledge in community nursing, oncology nursing, or emergency nursing is a plus.
- Current nursing license (RN) and CPR-C certification.
- Graduation from an accredited nursing program.
- Reliable vehicle and a valid driver’s license for travel to patient locations.

Compensation

- Part-Time: $55/hour
