This role includes creating a supportive environment for clients and directing the work of personal support workers and volunteers. You will engage with patients, implement care plans, and ensure that all documented practices are in line with Carefor and regulatory standards. Key Responsibilities:
- Perform detailed evaluations of client health
- Manage and implement individualized care plans
- Document responses and modify care plans as needed
- Provide health education to clients and staff
- Maintain clear communication with the healthcare team Requirements:
- Must be a licensed Registered Practical Nurse in Ontario
- Experience in clinical practice is required
