Job Summary

We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our dynamic healthcare team. Responsibilities

- Deliver comprehensive patient care by assessing needs, developing care plans, and implementing nursing interventions aligned with best practices.
- Administer medications accurately and safely, including injections, IV therapies, and pain management treatments.
- Monitor patient conditions closely, documenting changes and communicating updates to the healthcare team promptly.
- Assist with medical procedures such as phlebotomy (blood draws), wound care, and laboratory specimen collection.
- Utilize electronic health record systems like Epic or Cerner for documentation, order entry, and case management activities.
- Provide specialized care for patients with dementia, hospice needs, or requiring assisted living support.




- Collaborate effectively with multidisciplinary teams to coordinate patient services across hospital, home care, or community settings.
- Support patient education initiatives to promote health literacy and self-care strategies.

Requirements

- Valid registration as a Registered Practical Nurse (RPN) with the appropriate licensing body in your region.
- Proven experience in nursing home settings, hospitals, or acute care environments.
- Knowledge of medical terminology and medication administration protocols.
- Experience working with diverse populations including pediatrics, seniors, and individuals with dementia or hospice needs.
- Familiarity with electronic health records such as Epic or Cerner is highly desirable.
- Solid skills in patient care delivery, case management, pain management techniques, and laboratory procedures like phlebotomy.
- Ability to adapt quickly in fast-paced environments while maintaining a positive attitude and compassionate approach.
