RN or RPN – Primary Care (12-month contract)

Health Zone Nurse Practitioner-Led Clinic (HZNPLC) is a non-profit organization funded by Ontario Health. We are a primary care clinic led by Nurse Practitioners and supported by a full interdisciplinary team including nurses, social workers, a pharmacist and administrative staff. We provide care for our community in a culturally safe, inclusive and trauma-informed way, supporting both patients and providers in having a positive experience rooted in excellence.

We are currently looking to fill a temporary 1.0 FTE Registered Practical Nurse or Registered Nurse (contract) position, to begin October 1, 2026.

The successful candidate will work independently and interdependently as part of a dynamic interdisciplinary team, providing high-quality professional nursing care to patients, consistent with the vision of the Health Zone Nurse Practitioner Led Clinic.

Bachelor of Science in Nursing or a Diploma in Practical Nursing with current registration from CNO is required.

What you’ll need:

· Minimum one year of clinical experience as an RN or RPN, with preference given to candidates who have recent experience in primary care/community health, mental health, women’s health, and chronic disease management;

· Advanced knowledge and skills in current, evidenced based practices in primary care delivery with an emphasis on health promotion, risk reduction and injury prevention;

· Committed to working within an inter-qualified collaborative,



strengths-based, health promotion framework to address the barriers faced by marginalized populations is essential; previous experience working with marginalized populations is an asset;

· Self-motivated, flexible and creative approach to work and stimulated by taking on new challenges;

· Excellent interpersonal, communication, organizational, negotiation, and problem-solving skills;

· Current CPR certification

· Competency in PS Suites EMR and proficient with basic computer applications

· Competency in another language is an asset

· Evidence of professional liability insurance;
