Position Summary:
The Support Worker / Link Worker provides non-medical, non-clinical navigation, outreach, referral coordination, and follow-up support to older adults aged 55+ in Lloydminster and surrounding Alberta/Saskatchewan communities.
The role supports LRHG’s grant-funded Lodge-as-Hub Model, helping residents and non-resident older adults access community, housing-related, food security, transportation, income support, social connection, and healthy aging supports.
This position does not provide clinical care, personal care, home care, case management, tenancy enforcement, lease administration, rent collection, or eviction-related functions.
Temporary: 1 Year Tem (September 2026 – September 2027)
Key Responsibilities:
- Complete intake and identify non-clinical support needs.
- Provide service navigation and practical assistance with forms, applications, eligibility checks, online systems, appointments, and referrals.
- Coordinate supported referrals and warm handoffs to appropriate community partners.
- Follow up with clients and referral partners to confirm services were accessed.
- Support clients facing barriers related to transportation, digital access, system navigation, income supports, food security, housing stability, and social connection.
- Conduct outreach to LRHG housing sites, lodges, community partners, and rural communities within the Project Catchment Area.
- Build and maintain referral relationships with healthcare providers, municipalities, housing providers, transportation providers, FCSS organizations, food security organizations, and community agencies.
- Follow documented protocols for referrals, handoffs, escalation, information sharing, confidentiality, and role boundaries.
- Support cross-jurisdictional navigation between Alberta and Saskatchewan services where applicable.
- Track client activity, referrals, follow-up, service outcomes, partnerships, outreach, and engagement events for grant reporting.
Score of Services:
The Support Worker/Link Worker may assist clients to access:
- Housing-related supports and housing stability resources.
- Income supports, benefits, and financial assistance programs.
- Food security supports, grocery assistance,
meal programs, and meal delivery options.
- Transportation coordination for appointments, errands, services, or community activities.
- Social, recreational, educational, volunteer, and community participation opportunities.
- Light housekeeping, household management, light home maintenance, snow removal, or lawn care supports through appropriate service providers where available.
- Digital access, form completion, printing, appointment scheduling, and communication support.
Client Group The position serves older adults aged 55+ within the Project Catchment Area, including:
- Residents: Older adults living in LRHG lodge or lodge-operated housing.
- Non-Residents: Eligible older adults living in the broader community.
The Project target is to support at least 120 unique clients over the grant term. Grant Deliverables Supported by This Role The Support Worker / Link Worker will contribute to:
- Operational delivery of the Lodge-as-Hub Model.
- Approximately 8–12 active referral sources.
- Approximately 25–35 coordinated referrals per quarter.
- Support for at least 120 unique older adult clients.
- Strengthened referral pathways across health, housing, municipal, transportation, FCSS, food security, and community partners.
- Quarterly and final reporting on service volumes, referral outcomes, follow-up activities, partnerships, client supports accessed, and lessons learned.
Qualifications:
Required
- Post-secondary education in social services, human services, community development, gerontology, health navigation, or a related field; equivalent experience may be considered.
- Experience supporting older adults, low-income individuals, tenants, or people facing service access barriers.
- Strong communication, active listening, organization, and documentation skills.
- Ability to assist with online systems, forms, applications, email, printing, and basic digital navigation.
- Ability to maintain confidentiality, consent practices, professional boundaries, and accurate records.
- Valid driver’s license and access to a reliable vehicle for outreach.
- Criminal record check, including vulnerable sector screening, if required.
Preferred
- Knowledge of Lloydminster and surrounding Alberta/Saskatchewan community resources.
- Familiarity with seniors’ benefits, transportation services, food security programs, income supports, FCSS programs, housing supports, and healthy aging services.
- Experience with referral coordination, outreach, warm handoffs, community hub models, or grant reporting.

- Transportation coordination and enhanced support activities.
- Services and supports accessed.
- Outreach, engagement events, and partnerships.
- Service gaps, barriers, lessons learned, and early impact observations.
Working Conditions:
- The role will be housed out of Pioneer Lodge.
- Work occurs at LRHG sites, lodges, community hub access points, partner locations, and outreach settings.
- Local travel throughout the Project Catchment Area is required.
- The role may involve supporting clients experiencing isolation, transportation barriers, financial hardship, housing instability, digital barriers, or complex service needs.
- Some schedule flexibility may be required for outreach, meetings, or engagement events.
