Key Responsibilities:
• Perform scheduled community visits for patient assessment
• Deliver ethical nursing care with patient safety as priority




• Implement and monitor effective care plans
• Foster relationships by collaborating with patients
• Document and update patient records thoroughly

Requirements:
• Active RPN registration with the CNO
• Driver’s license and vehicle access mandatory
• Community care experience preferred
• Reliable and adaptable availability
• Prepared for after-hours on-call duties
