Key Responsibilities:
Complete home evaluations within 24 hours of referral
Develop individualized treatment strategies and track progress
Educate clients and families on safety and equipment




Document all assessments and communications clearly
Coordinate care with an interdisciplinary team to optimize outcomes

Requirements:
Experience in home/community care strongly preferred
Registered with the College of Occupational Therapists of Ontario
Proficiency in safety evaluations and ADL retraining
Valid driver's license with reliable transportation
Ability to manage independent schedules and documentation
